Every year
thousands of individuals are hospitalized or require some form of emergency
room treatment, procedures that almost always cost much more than a simple
doctor visit. To help offset these expensive and often time consuming
visits, you can cover yourself with a Hospital Cash Plan.
This plan is designed to pay you a set amount of money due to events such
as hospital confinement or emergency room treatment.
Benefits:
Hospital Confinement Benefit: Humana will pay $500 for each
hospital confinement of a covered person, no more than once per person per
year. Confinements for the same or related injury or sickness will be
considered a single hospital confinement unless separated by a period of at
least 180 days.
Emergency Room Treatment Benefit: Humana will pay $150
when a covered person visits a hospital emergency room and requires and receives
emergency room care. No benefits are paid for more than two such visits
for each covered person per year. For a Couple plan, no benefits are
paid for more than three such visits per year, four visits per year for Single
Parent plans, and six visits per year for Family plans.
Outpatient Surgery Benefit: Humana will pay $150 when
a covered person requires and undergoes an outpatient surgical procedure for
which a fee is charged. No benefits are paid for more than two such
visits for each covered person per year. For a Couple plan, no benefits
are paid for more than three such visits per year, four visits per year for
Single Parent plans, and six visits per year for Family plans.
Exclusions and Limitations:
Coverage for pre-existing
conditions does not take effect until twelve months after start of coverage.
Exclusions and limitations include cancer during the first six months, pregnancy/childbirth
during the first ten months, self-inflicted injury, or treatment due to being
under the influence of drugs or alcohol. For a full list of exclusions,
